If true layout is important to you, there is a, rather expensive, SONY ebook reader, that is specialized on PDF. If you want to consume PDF on an eBook reader the best options are to flash an alternative firmware with strong PDF support or going 10" screen size with your reader. PDF on eBook readers is a pain in the #*?!. You most probably have to do a lot of rescaling by hand, since PDF, usually, comes either in US Letter or DIN A4 formats, which will not match your eBook reader's screen size. I think Calibre has some function (in the main program) to rescale pages to fit your eBook reader screen. Both can import HTML and help you make it an ePub. Now, that you have your HTML and assets you may use either Calibre's Ebook Editor or Sigil. Either both of these, or at lease one of these is capable of retaining the original layout, if memory serves me correctly. Since it is built on Java it runs cross-platform, as long as you have Java installed. The supported documents that you can import into the application. There is also PDFBox which also can convert PDF to HTML. The IDEAL ePub Creator app is tailored to converting only document files to the EPUB format.

If you want to run it on Windows, you must go the Cygwin route and install the Cygwin package. There are some command line utilities coming with it, that will do the conversion from PDF → HTML. There are distributions for all major operating systems. Poppler is a PDF rendering library based on the xpdf-3.0 code base.
